Standing Delegation PowerPlans Title Change Approved

Use and Standards Committee has approved the changing of Standing Delegation PowerPlan titles to include “SDO” and the physician name. For example: “Rapid Response Plan” will become “Rapid Response SDO- Dr. Alalawi”. This will ensue that the nurse knows the correct communication type and the correct physician name to place the orders under. Performance Improvement has confirmed that the following physician signatures are on file for the corresponding plans and therefore will be updated with new titles:

1.       Wound Care SDO- Dr. Griswold

2.       Rapid Response SDO- Dr. Alalawi

3.       Newborn Nursery SDO- Dr. Robinson

4.       NS PowerPlan SDO- Dr. Ragain

5.       Anesthesia Pre-Op Screening SDO-Dr. Johnson

6.       Sane Examination SDO-Dr. Piel (EC use only)

7.       Physical Therapy Topical SDO-Dr. Griswold

NS For Procedure Setup PowerPlan

DO NOT DISCONTINUE NS FOR PROCEDURE SETUP POWERPLAN OR INDIVIDUAL NS ORDERS
 
To assist the nursing staff in charging for NS that is pulled out of the pyxis for procedural use (blood infusions, IVPB infusions, CRRT, etc) the nurses now have a NS FOR PROCEDURE SETUP PowerPlan containing 3 NS orders (250 mL, 500 mL, and 1000 mL bags of NS).    They need these orders to remain active throughout the patients stay.  The plan is initiated under Dr. Ragain as he is the champion physician for this process.   Please help us ensure that our nurses have the necessary orders to carry out their workflows.
 
If you are doing TRANSFER RECONCILIATION, you can easily see that the orders are part of this necessary PowerPlan by hovering over the PowerPlan icon.  Again, please help us by leaving these orders alone and NOT discontinuing them.

Outpatient Rehab Services Outpatient Dietician

Physicians,

We now have an Outpatient Dietitian available to provide counseling to your patients in need. If you have a patient in the hospital that would benefit from additional medical nutrition therapy as an outpatient, you may refer them to our services through Powerchart with these easy steps.

  1. Order Social Services for Outpatient Rehab Services
  2. Click Outpatient Dietitian
  3. Specify in the Special Instructions the reason for the Outpatient visit. Examples include: Diabetic Diet Counseling, Weight Management, Heart Healthy diet instruction, Celiac Diet, etc.
  4. Social Services will visit the patient while they are inpatient and arrange for outpatient nutritional services.
  5. The outpatient dietitian will assess and counsel the patient at the arranged time and date and document the visit in Powerchart.

Radiology Reports

Radiology Reports

We are aware that some radiology final reports appear blank, and we are actively working with Cerner on this issue.   The text is within the report, but is displaying white in color.  

 

A shortcut to view these reports:

•Click on the Windows “start” menu.

•If note pad is in the selection, double click to open the application.

•If it is not viewable, type “notepad” in the search window.

•Once open, navigate to the radiology report within flowsheets.  

•Highlight all of the text and using keyboard shortcuts select Ctrl + C to copy the selection.

•Place your cursor within notepad and select Ctrl + V to paste the selection.

 

The report should now be readable and in black font.

We apologize for this inconvenience and working to resolve as quickly as possible.

Related Records

Providers will now have the ability to access related records specific to mother and child (newborn).  This will only be applicable for mothers that delivered at UMC.

While in mother’s or newborn’s chart, click on the Related Records button found in the top Organizer bar (where the Exit, Calculator, and Explorer Menu are located).  When this button is clicked, the linked chart will automatically open.

Delay in Discharge

If you have initiated the Discharge Powerplan and your patient’s discharge is delayed or placed on hold, DO NOT discontinue the entire plan.   Simply find the Discharge patient order in the plan and right click and Cancel/DC the individual order.  Don’t forget to communicate with your nurse that you have done this.   When the patient is ready to go home, reorder the Discharge patient order.  This will prevent having to redo the entire plan and will ensure that no orders get missed, thus better utilizing your time and ensuring patient safety.

NOW and STAT Orders

If you are ordering a medication that has a scheduled frequency (Daily, BID, etc…), the start time on your order will default to the next scheduled time unless you choose a NOW or STAT priority.  For instance, a Daily order that is placed at 10:00 am will default to tomorrow at 9:00 am because that is the next available scheduled time. 

To prevent missed doses, choose a priority of NOW or STAT using the field illustrated below.  That will place an additional dose on the MAR/MAW for nursing staff at the current time and pick up the ordered frequency schedule at the next available scheduled time.  So, in the example above, placing the Daily order at 10:00 am with a NOW priority drops a dose on the MAR/MAW at 10:00 am and the next one at 9:00 am tomorrow.

Fluid Orders on OR/Cath Lab Patients

FLUID ORDERS ON PATIENTS COMING FROM OR/CATH LAB

During the week of May 12th –May 15th the inpatient nursing staff have been moving to a process known as Auto Pump Programming.  This allows the nursing staff to program their medications on the pump versus manually programming them to help remove potential keying errors.  If the patient comes from the OR or CathLab with a medication or continuous infusion already infusing, it is very important that the nurses have orders for these medications/fluids. 

If you wish for any mediation to continue being infused once the patient is on the unit, please make sure that orders are in place that guide the nurse to ensure they give all the proper care needed.

Providers and Nurses:

In preparation for the transition to ICD-10 it is important for all providers and nurses to attend one of the 38 training sessions listed below.  Participants will learn the phases to improve documentation using ICD-10 including the ability to use the diagnosis control to consistently insert diagnoses in notes.  Training sessions are provided for all times of the day to accommodate your busy schedules beginning as early as 7:00 a.m., through the lunch hour (first 25 to arrive at the door will receive a free lunch), and as late as 4:00 p.m.
