PowerChart Controls for UMC Nursing Staff

Please be aware that starting on May 20th, UMC’s IP, OP, and EC nurses will begin using the Powerchart Controls for problems, procedures, and family history to document the corresponding intake items.  This is the next step we are taking to get all past medical history documented consistently in one area during intake.

UMC nursing staff will follow the same workflow as clinic staff in that problems will be documented as ‘Patient Stated’ when obtained during the intake process.

University Medical Center Health System, Physicians Network Services, and Texas Tech University Health Sciences Center are providing you with web-based ICD-10 education from HCPro and Cerner.  Further education for Cerner PowerChart education will soon follow.  Please follow the instructions below to access your education and earn free CMEs

This message is to let you know that you have been given access to Learning Exchange (LX) – your online leaning management system for ICD-10 training from HCPro and Cerner.   

GETTING STARTED:

     *Go to http:lx.learningmate.com/hcpro

     *You will be directed to a login screen

           Enter your email ID addresss:  yourfirstname.yourlastname@umcicd10.com

     *Once logged in, you can begin learning

     *Note: You can change your password by clicking on your name in the top right corner and

                 selecting "profile" from the drop-down menu.  Select "edit profile" and then click

     *Click on the "courses" tab and select "my courses" to view your list of online courses.

     *Click the title of a course and then select "start studying" to view each individual course.

     *You should complete the introductory sessions first.  Start with the Introductory training

       course, followed by the General Medicine or Surgery course, before proceeding to the

       other courses.

Internet Explorer 11 currently has unresolved incompatibility issues involving eLearning content. To view the content within IE11, Compatibility View settings must be updated.

To change your Compatibility View settings

  1. Open Internet Explorer for the desktop, click Tools, and then click Compatibility View settings.
  2. In the Compatibility View Settings box, add the problematic website URL, and then click Add. Compatibility View is turned on for this single website, for this specific computer.
  3. Decide if you want your intranet sites displayed using Compatibility View, decide whether to use Microsoft compatibility lists, and then click Close.

Communication Control To Intake Forms

In order to meet Meaningful Use (MU) criteria for patient reminders, we have added the Communication control.  The MU objective states that reminders must be sent to the patients per their preference, such as a phone call, letter, or the patient portal.

The Communication control will be added to the Communication section of the existing General Information tab on all clinic intakes (for PNS, it will be the Ambulatory Intake - PNS).  It is not a mandatory field but must be filled out and select the “Submit” button to receive credit.  If the patient is not signed up for the portal, the “Patient Portal” option will remain dithered, leaving you only the choices of “printed letter” or “phone call”.

High Potassium Alert

* Alert ordering staff when a potassium supplement is ordered and the latest potassium

   level is 5.7 MMOL/L or greater.

* Patients 12 or older

* The alert is "encounter specific", meaning the lab ordered and resulted on the same

    inpatient visit will fire the alert.

This new rule will only fire from a potassium lab level and not the POC potassium.  This was approved by the Use & Standards committee.

Time Sort Display in INet

At Use and Standards the group decided to change the time column display in INet to display in Reverse Chronological, which would make it standard with the Flowsheets.  This setting will only affect the band names that begin with Physician.  The other bands were designed for nursing and they prefer the chronological display.  This change will occur on April 8th.  See upcoming changes below.

 

 

Enoxaparin Auto Rounding

Enoxaparin auto rounding was approved by the Use and Standards Committee last week.  This process will be turned on April 15th.

Decision: Do we enable auto-rounding in the dose calculator for Enoxaparin?

Recommendation: To improve dose range checking, Enoxaparin should be auto-rounded in teh dose claculator within PowerChart if the option for 40 kg or GREATER is chosen

Social Services Orders

Please be advised that as of April 6th, social workers will be ‘completing’ their social services orders once the order has been received.  This does not mean they are finished with the set-up, but that it is being processed.  

 We would like to reinforce to providers that it is very important to get your Consult for social services orders in as soon as you are aware of any needs so that these can be processed ASAP to help prevent delays in patient discharges.

Radiology Diagnosis Changes

Diagnosis Requirement for EKG, Cardiovascular, & Cariopulmonary Orders

Effective September 24th, 2015:  Diagnosis will be required when placing ALL EKG, Cardiovascular and Cardiopulmonary orders.  This is an expansion of the original conversion from March 25th, seen below.

***When giving orders via phone or verbal, a nurse will prompt for a diagnosis.***   

Focus should be to underlying symptoms and reason to perform the exam, not the interpretation.

Effective March 25th, 2015:  Diagnosis will be required when placing ALL Radiology orders both Inpatient and Outpatient. When giving radiology orders via phone or verbal, a nurse will prompt for a diagnosis. 

There is a print-out (in blue) at the bottom for more in depth education if needed.

 

*Problem/Diagnosis control will be available in the orders window

All admit plans WILL require an actual diagnosis in the request bed order and will populate the diagnosis control.  If an appropriate diagnosis exists, click the check box and continue placing the radiology order/powerplan.

*Select only the applicable diagnosis for the radiology order

 

Choosing a specific diagnosis (not all) will assist the technicians and radiologist perform the exam.  Reason for Exam will not be required.

Cardiovascular Results to Message Center

Effective March 10th, 2015:  Cardiovascular finalized results will be sent via a “message” to ordering providers within your message center inbox.

NEW - Elderly Alert

An Alert will pop up when medication is added to the scratchpad when the patient age is greater than 65 years old and one of the medications identified by the Clinical Decision Support committee is prescribed.  Options to cancel or continue with reasons available.  Beers Criteria (button) will open to a pocket guide for medications and reasons.  A copy of the Beers Criteria can be provided upon request.

A new alert will fire when a patient is less than one (1) year of age and Ceftriaxone and/or injectable calcium is ordered within 48 hours of each other.

Non-Provider Alert will be allowed to cancel and override with message to contact the provider to give an override reason.

Blood Product Ordering Update

STARTING, March 15, there will be a change in blood product ordering for all products, which include red cells, plasma, platelets, and cryoprecipitate.  The “BB On Hold” orders will have a name change to “BB for Pre-Op.”  This order will be used for only pre-op or pre-procedure blood product orders and you will be required to indicate the date needed.  The “BB To Transfuse” orders will be used for “Emergent” orders or blood that will be transfused that day. 

New procedures in the transfusion service allow for crossmatching and issuing blood within 1-3 minutes, once a negative type and screen is complete.  Patients with antibodies will have 2 units automatically held until the clot expires. 

 

Security Updates at UMC - CD/USB Usage

Due to HIPAA and HITRUST requirements, all external media (CD/USB) used at UMC will now be encrypted.  IT Security will be pushing out a new security product called Cryptzone to your department’s computers.  Cryptzone is designed to encrypt USB devices such as thumb drives as well as CD/DVDs.  When the media is plugged into a computer it will display the following:

 

 

By clicking “Continue” the drive will be encrypted (secured) and the user will be prompted to setup a passcode for the CD/DVD or thumb drive.  If the user clicks “cancel” they will still be able to read and remove data but not write to the device.
