Medication History Snapshot

  • Provides static views of a patient’s home medication list at known points in time
  • Ability to compare the patient's home medication list at the time they were admitted to after discharge
  •  View home medication changes from recent Inpatient visits
  •  See how the patient’s home medication treatments may have changed over time (for example, dosage changes.)

New Medication History Snapshots are created when any of the following actions are taken:

  • A prescription or documented home medication order is signed
  • Home medication compliance status is added or modified
  • The Document Medication by Hx conversation is signed (includes No Known Home Medications or Use Last Compliance)
  • The medication history status is reset
  • Discharge Medication Reconciliation is signed

Accessible from all PowerOrders navigator views & Documenting Medications by Hx:

Select Orders from the Menu on the left hand side and the Medication History Snapshot is below Medication History

Click Document Medication by History and expand the Document Medication by Hx button:

The Medication History Snapshot will now be visible

Each snapshot has a header row that specifies the date and time it was generated as well as the user who took an action that resulted in the snapshot being generated

The Medication History Snapshot only includes the list of documented home medications and prescriptions that were active at the time the snapshot was created

Inpatient and ambulatory in-office medications are NOT included

You can change the time frame by selecting This Visit, 6 months, 1 year, or All Visits.

Snapshots can be expanded by clicking the black triangle:

Clicking an order will open the Order Information screen for that specific order

External Rx History

The External Rx History function in PowerChart is designed to let providers see what medications a patient has filled at retail pharmacies. This can help with the patient’s home medication list by providing a way to verify the dosages or frequencies of a prescription if the patient cannot remember. It can also help identify if a patient is attempting to get another narcotic prescription or fill one too early by showing the prescription and the fill dates.

There are a couple of caveats to this system. If a patient pays cash or chooses not to run the claim through insurance, the fill information may not be transmitted from the external pharmacy back through SureScripts into Cerner. Also, if the pharmacy does not adjudicate claim information or does not participate with SureScripts, there may be no data.

MAR Summary from Previous Encounters

Ability to View Medications Given in Previous Encounters

We have introduced a new feature in Powerchart to make it easier to view medications given on a previous visit.  This will be useful in the event that a patient had medications given in the EC, SWCC, etc. and then admitted as an inpatient a few days later.

To view this history:

Navigate to the MAR Summary from the Menu tab:

Right Click on the gray bar and choose “Change Properties”, adjust the date range to go back to the previous visit you wish to view.   You will now be able to view the date/time of any meds that were given.

To search for previous visits:

Click on the Patient Location on the banner bar to view a list of previous visits.

In the example below, the patient was admitted to observation on 1/14/2016, by clicking on the location button, a list of previous visits appear.   This patient was also admitted to the Emergency Center on 1/6/2016.  To view what was given on the EC visit, the date range on the MAR Summary was changed to 1/6/2016 and now you can view what meds were given in the Emergency Center on that visit.



How to Change the EMAR Search Criteria

The default may be alphabetical; however, the user can change the setting by right clicking on the timeframe, choosing Change Search Criteria, then changing the Order Sort to Sort by next task due. 

Make sure that the Save as default sort is checked before clicking OK. Once this is set, the EMAR with default to a sort of next task due.  Below is a screenshot on how to make the preference change.

 

How to Renew a Prescription in PowerChart

For your next ePrescription, try using the “Renew” function for the existing prescription already in the system. 

  1. It is easy to do
  2. It keeps your patient’s medication list clean, no duplications
  3. And it can track how many times that medication has been ePrescribed/refilled.
