ORDERING FOLLOW-UP APPOINTMENTS FOR DISCHARGE
Follow-up appointments should be entered as a separate appointment order for each provider/service the patient needs to follow-up with. Below is an example follow-ups ordered correctly.
In the example below staff can’t tell if they are following up with the surgeon, the PCP, or podiatrist or when to schedule follow up appointments.
If the patient needs more than 3 follow-up appointments, more can be added by clicking ‘Add to Phase’ and ‘Add Order’.

All Orders Transfer Reconciliation
Transfer Reconciliation is available in the Reconciliation button where the Admission and Discharge reconciliations are. All orders will show in the transfer reconciliation screen instead of just medications. There will be NO green check mark for Transfer because not all patients will be transferred during the stay.
There are check boxes for all catalogs except for medications so that all orders can be continued or not continued with one click. All current inpatient orders will default to Continue. The patient’s home medications that were NOT continued on admission will default to Do Not Continue. However, this gives you another opportunity to review the home med list and restart home medications as needed.
