Ambulatory Fall Risk Assessment –
The Fall Risk Assessment will be added as a section to the all ambulatory intake forms. This form is based off the CDC STEADI Algorithm for Fall Risk Screening, Assessment, and Intervention and the Risk Recommendations field comes from the recommendations of the STEADI algorithm.
